چکیده

Although basket impaction rarely occurs during retrieval of common bile duct stones, it is a potentially serious event [1]. The Trapezoid RX wire-guided retrieval basket (Boston Scientific Corp., Marlborough, Massachusetts, USA) has an emergency release feature, whereby the tip is designed to disengage, thereby facilitating basket removal and reducing the risk of impaction [2]. However, we encountered a rare case of trapezoid basket impaction that was complicated by difficulties in detaching the outer sheath from the basket wires. A man in his 50s was admitted with multiple common bile duct stones (●" Fig.1). After endoscopic sphincterotomy and endoscopic papillary large-balloon dilation, mechanical lithotripsy was attempted using a Trapezoid basket. However, basket impaction occurred because the stone was too hard and the tip of the Trapezoid failed to disengage. We attempted to remove the duodenoscope and outer sheath simultaneously, but were unable to remove the outer sheath at the stomach because it was entwined with the inner basket wires. As a result, only the duodenoscope was removed, following which a forward-viewing endoscope was inserted along the basket catheter. Using rat-tooth forceps, we grasped, twisted, and pulled the basket wires into the working channel of the scope. Next, we simultaneously pushed the scope to the anal side and pulled the outer sheath to the oral side, dissociating it from the basket wires, so that it could be successfully removed (●" Video1). The impaction was subsequently successfully reversed using an emergency mechanical lithotriptor handle (Olympus Medical Systems, Tokyo, Japan). Therefore, even when using a Trapezoid retrieval basket, basket impaction can occur and an emergency mechanical lithotriptor handle should be available on standby. Additionally, entanglement between the outer sheath and the inner basket wires can occur because the outer sheath of the Trapezoid is thinner than some other mechanical lithotriptors. The technique described here may be useful in such cases.

Intraductal cholangioscopy-guided electrohydraulic lithotripsy as a rescue therapy for impacted common bile duct stones within a Dormia basket.

The treatment of bile duct stones has evolved from surgery to endoscopic management, which has a success rate of 90% [1]. Impaction of the lithotripsy basket during endoscopic removal of a common bile duct (CBD) stones is seen in up to 5.9% of the cases [2,3] and represents one of the most feared complications; as it usually results in open surgical intervention. We report our experience with a...

Argon plasma coagulation for the resolution of basket impaction from large common bile duct stones

One of the most common indications for endoscopic retrograde cholangiopancreatography (ERCP) is choledocholithiasis – the occurrence of stones in the common bile duct (CBD). However, in approximately 5% to 10% of cases, the procedure fails, especially in a patient with large or impacted stones [1]. An uncommon adverse event during ERCP with CBD stone clearance is impaction of the basket-stone c...

Percutaneous Transhepatic Release of an Impacted Lithotripter Basket and Its Fractured Traction Wire Using a Goose-Neck Snare: a Case Report

In a patient with a distal common bile duct stone, a fracture of the traction wire of the basket occurring during the performance of mechanical lithotripsy resulted in the impaction of the lithotripter basket with a stone. The impacted lithotripter basket combined with a fracture of the traction wire is a rare complication of endoscopic stone removal. We were able to pull the impacted basket us...

Laser lithotripsy to treat basket impaction during mechanical lithotripsy of a pancreatic duct stone.

CONTEXT Though uncommon, pancreatic duct stones can cause significant discomfort and morbidity. Endoscopic removal of pancreatic stones may decrease intraductal pressure by augmenting ductal drainage. Endoscopy is shown to be most effective when used early in the course of the disease. Endoscopic methods such as mechanical lithotripsy are often successful in removing the majority of pancreatic ...
